Hospice Services

Our hospice and palliative care service focuses on individualized medical nutrition therapies at the end of life. Our compassionate, caring dietitians work with your Interdisciplinary team to help clients and families deal with nutrition and hydration concerns and decisions.We will attend care plan meetings, provide in-servicing for your staff, and provide educational support and materials for your clients either through phone or in-home consultation .

Goals of Hospice Medical Nutrition Therapy: The primary goal of nutrition and hydration for our clients is comfort. Our staff plays an important role in enhancing the quality of your client’s lives by treating every client with understanding and patience when nutrition and hydration decline. Our purpose is to maintain, or optimize the client’s intake to provide comfort and enjoyment.

Home Health Care Services

NCS Dietitians can schedule in home visits for your clients to teach nutrition and hydration therapies for those with needs in the home setting. We can help with tube feedings, home IV use, ostomy and wound healing. Diet instruction and materials can be provided in the home by one of our skilled staff dietitians.

Goals of Home Health Medical Nutrition Therapy: Our primary goal is to improve or maintain the nutrition and hydration status of individuals in the home health setting by supporting the client and the family with education and training based on common nutrition goals of the client and physician.
If needed, consults can be done via phone and materials can be delivered by staff that visit on a regular basis.
